About the CEC Treatment at the CEC What is Endometriosis? Educational Library Patient Forms |
Sending Your RecordsIf you feel, after examining our materials, that you would like to pursue treatment with a CEC-accredited physician, you can send your records in for evaluation. Drs. Albee and Sinervo will respond personally, providing you with their comments and outlining a treatment plan for you. To send your records in for review, please first register with the CEC by visiting our short information request form at http://www.centerforendo.com/sendinfo.htm. Once registered, our Program Director will contact you with further information, including details on how send your medical records in to the CEC. Alternatively, you may also contact our Program Director directly at 866-733-5540 or via email. It is imperative that you first register with our Program Director before sending any records, in order for us to better track your case. Because the doctors do not charge for this records review service, they ask that only women who are seriously considering treatment here submit their files. You do not need to have had previous surgery or have an advanced case of endometriosis to be considered for treatment at the CEC. If you know you have Endo, or if you think you might, we are happy to try to help you. For records review and evaluation, the doctors will need the following items, all records of previous pelvic surgery (if any), including:
Tapes and photos are returned when the doctor completes his evaluation.
The
doctors can respond soonest if you arrange all materials in
chronological order. They cannot respond until all materials are
received. Please do not send partial files, as we cannot track them
