NEW REGISTRANTS PLEASE NOTE:  Each and every registration form submitted to our offices is responded to via an email message back to the address you provide us with, within 2-5 business days (those without email will be sent a packet via U.S. postal mail).  Please take care to input your email address correctly, double-checking that you have given us accurate information.  We cannot reach you if you provide us with an email address that does not work or does not accept email from us.  Hotmail, Yahoo, GMail and other “free” email account users PLEASE ALSO BE ADVISED:  Many such users contact us after registering to state that they have not received a response from us.  Please be assured a response was sent to the address you provided.  Unfortunately, many such “free” email service providers do NOT deliver incoming messages from the CEC.  Check your spam filters and be sure to add Heather(at)CenterForEndo(dot)com to your accepted addressee list in order to ensure that you do receive the important and helpful information we send out to you.  If you are expecting an e-response from the CEC, be advised it has been sent; if you do not receive it within 2-5 business days, it has been bounced by your server and we have no way of contacting you further (we do not make unsolicited phone calls).  Please contact us toll free at 866-733-5540 to provide alternative delivery information as soon as possible.  Thank you for your help and consideration on this important matter.  We want to be sure to assist everyone, and your attention to these details will help make that a possibility.  We look forward to talking with you!

ALL PATIENTS PLEASE NOTE: If you are sending videos along with your case file for us to consider, be advised that the tape MUST be properly labeled with your full name, date of birth and phone number.  Videos which do not bear this information will NOT be considered during the case review.  Thank you.

All fields requested below are required.  Completing the information as requested allows us to register you and receive your files for evaluation.  Without all the information, we cannot process your registration accordingly.  If you do not have email access or a cell number, or it is not permissible to contact you at work, please simply note “N/A” in those fields.  If you have an automatic spam blocker (i.e., Earthlink accounts), please add “CenterForEndo.com” to your approved list of domains.  Also be sure to double-check your email address input, as incorrect information in this field will result in an undeliverable response from us.  We cannot and will not be responsible for messages not received as a result of incorrect information or blocked accounts.  Please be advised that the CEC places the highest value on your privacy; this information is collected of potential patients solely in order for us to better meet your needs.  This data is held in the strictest of confidence and is never sold, shared or provided to any third party for any reason, at any time.  Submitting this information in no way obligates you to, and is not indicative of, a doctor/patient relationship.  Thank you for helping us to ensure accuracy in our records and to meet your information request in a timely manner.

PLEASE BE ADVISED:  The CEC will return medical records upon request at no charge.  Should you require the return of your case file, please contact us at 770-913-0001 or include a note indicating your request in your records review file.  Please be further advised:  we purge files one year out from receipt of records.  The records of those individuals who do not become CEC patients within one year of sending their case for our free review and evaluation will be destroyed, unless they specifically request return of those materials within the one year time frame.  Thank you.
