Procedure for patient referral
Prospective patients must submit relevant medical
records, tapes, photographs, etc., along with a written narrative of current
symptoms. These materials shall be sent directly to the physician they
choose. Physicians must inform the Program Director of the records they
receive, in order that adequate records are maintained.
Physicians will follow-up with every patient
referred to them. If a patient is not considered acceptable for surgery for
any reason, the physician shall communicate this reason to her. The patient
may then be referred to another CEC physician, if she desires. |
